Jessica Bryant, RN, earned an Associate’s Degree in Nursing from Angelo State University and is a certified instructor for Advanced Cardiac Life Support. As a member of Cardiac Rehab team for two years, she has experience providing care in long-term acute care and intensive care settings. One of the things she loves most about nursing is that there are so many ways and opportunities to help people. She truly loves being able assist people in maintaining and even improving their quality of life. Jessica lives in the Brazosport Area with her two sons and daughterut EKG’s. Amanda lives in Lake Jackson with her husband Michael.





Allison Cavanaugh, MSis originally from San Antonio, TX. She received both her Bachelor & Master’s degree in Exercise Science at Texas State University. Prior to joining our team here at CHI, Allison and her husband, Trey, spent a couple years traveling across the country living in several different states due to her husband’s career. While traveling she held several different positions in the health care field. In her free time, she enjoys playing with her two dogs, Tessa and Ace, and exploring new places with Trey.

Allison is an exercise physiologist in the cardiac rehab department where she enjoys working with the cardiac and pulmonary patients towards achieving their healthier functional lifestyle. 

She is very passionate about educating and helping her patients achieve their goals. She is currently working towards her degree in the nursing program at Brazosport College. Her career goals are to become a nurse with a specialization in the rehabilitation/orthopedics field. 


Sherry Lynn Carag graduated from University of Baguio, Philippines in 2002 with a Bachelor’s degree. She completed her Master’s in Physical therapy in 2014, from University of Santo Thomas. She relocated to United States in 2016, pursued her doctorate degree from Utica College New York and successfully completed in 2018. She has 13 years of experience providing care to patients with musculoskeletal, neurological and pediatric conditions.  She has a great interest in neurological and pediatric patients.

Sherry began her career in the U.S with CHI St. Luke’s Brazosport in 2016. She continues to advance professionally with taking several continuing education courses. She believes in evidence based practice combined with patient centric individualized treatment. Sherry focuses on being a patient advocate, and assist them to reach their maximal functional potential to lead a healthier life style.

Sherry enjoys traveling, reading books, watching television especially football and baseball (Astros and Cowboys), and relaxing with her friends. Sherry also enjoys learning how to cook, home improvements and gardening in her spare time.

Peyton Cole is a born and raised Brazoria County resident. She attended Brazoswood High School where she graduated with her degree in 2011. Peyton has been with CHI St. Luke’s Health Brazosport for 4+ years, working her way up from the Environmental Services department to the Supply Chain department, and has now been working for the Outpatient Rehab Department for a year and a half as an Administrative Therapy Assistant in our front office.


Peyton lives in the area with her significant other, Anthony, and their 2 daughters, Avianna and Amiyah. In her free time, Peyton enjoys attending her daughters’ softball games, Anthony’s little league team’s Football Games, cooking, and outdoor family activities. She definitely wants to stay in the Healthcare Field and has taken a few health science classes at Brazosport College. She plans on continuing her education once she decides what area she sees herself working in long term, but for now enjoys interacting with her patients from the front end.


Dan Diltz has 15 years of experience as a licensed Physical therapy assistant (PTA).  Dan graduated from Rhodes College in Lima, Ohio with a degree in applied science in 2003.  Dan also has an associate degree in accounting from Northwestern Business College in Lima, Ohio.  Dan’s work experience as a PTA includes home health, aquatics, long term skilled healthcare, behavioral units, Texas Department of Corrections Transitional Care Unit, UTMB’s Blocker burn unit, Outpatient rehab, Inpatient rehab, and acute care hospital settings.  Dan has had strong outcomes working with patient’s that have gait, strength, and balance deficits.  Dan enjoys being activity involved with the Central Assemblies of God church in Angleton, Texas.  Dan has also volunteered for the Lake Jackson SPCA humane society.  Some of Dan’s favorite activities include spending time with family, hiking, travelling, and being outdoors.

Bailey Hammack, born and raised in Danbury, Texas, Bailey Hammack has been interested in physical therapy from a young age due to her participation in multiple sports. Her sports career led her to competition at the collegiate level in softball at Texas Woman's University (TWU) in Denton, Texas where she was nominated as a Finalist for the NCAA Woman of the Year in 2014. While at TWU,  she also received her Bachelor of Science in Kinesiology-Exercise Science. Due to her involvement in sports, exercise science, and rehabilitation Bailey has grown to become passionate in pursuing a career in physical therapy in order to utilize her skills/knowledge to help people reach or return to their full physical potential. She went on to TWU Institute of Health Science Center in Houston, Texas to complete her Doctorate of Physical Therapy, which she received in May 2017. Since graduation, Bailey's interest in Orthopedic Physical Therapy grew with participation in the Harris Health Orthopedic Physical Therapy Residency program, in which she was able to expand her knowledge/skills and receive mentorship from residency and fellowship trained physical therapists. This experience helped Bailey develop the level of expertise she desired rooted in Orthopedic Physical Therapy, with enhancement of manual skills, in order to provide evidence based care and centered around community health. Currently, Bailey is an active member of the American Physical Therapy Association with interest in working towards her certification as an orthopedic clinical specialist, future fellow of manual therapy, and certified clinical instructor. Bailey is also interested in sports rehabilitation and is currently working on the development of a Knee/ACL Injury Prevention Program for our local adolescent athletes. Bailey has been a resident of Brazoria County from birth and is grateful to be given the opportunity to share her skill set with the community that made such a huge impact on her life.


Premila Martin has been practicing as a physical therapist since 2002. She graduated from Padmavathy college of physiotherapy with a bachelor’s in physical therapy. She completed her doctorate in physical therapy from Des Moines University. She is a board certified Orthopedic clinical specialist. Premila is also certified in Manual Therapy and a credentialed clinical instructor. She is currently pursuing her master’s in healthcare administration. She enjoys treating a variety of conditions but has a special interest in treating musculoskeletal orthopedic conditions and concussion. She also specializes in treating patients with vestibular dysfunction. She is a patient advocate and strongly believes in improving patient awareness and promotion of overall health and wellness. She believes in an individualized patient centered evidence- based treatment approach. In her free time, Premila enjoys gardening, watching basketball and football games. She loves spending time with her family and traveling.

Aleyda Ramirez, Exercise Physiologist, received a Bachelor's in Science from Texas State University in 2013. She has been with CHI since 2017 and previously interned with St. Luke’s at the Texas Medical Center in non-invasive cardiology. Her interest includes health and wellness promotion and cardiac rehabilitation. Aleyda and her family currently reside in Lake Jackson.


Cynthia Rice, PTA, received an Associates of Applied Science from Wharton County Jr. College in 2003. She has worked as a licensed physical therapist assistance since 2003 and has been a Registered Massage Therapist since 1997. Cyndi has worked in the outpatient therapy clinic with Memorial Hermann Southwest and Dynamic Rehabilitation in Pearland before joining Brazosport Regional Home Health and Outpatient Rehabilitation. Her favorite treatment area includes outpatient orthopedic and sports medicine. Cyndi and her daughter currently reside in Brazoria




Rhonda Shepard, PTA, received an Associate of Science from Brazosport College then continued her education path earning an Associate of Applied Science form Wharton County Jr. College in 1995.  She has served the Brazosport community and surrounding populations since completing the Physical Therapist Assistant program at that time.  She has 23 years of experience in outpatient rehabilitation.  Prior to joining CHI St. Luke's Brazosport, Rhonda provided outpatient services in several other local clinics allowing her the opportunity to meet and help many local residents.  Her years spent in the outpatient rehab realm has resulted in experience with Orthopedics, Sports Medicine, and Aquatic Therapy.  Her special interests include orthopedic conditions and sports medicine.  She enjoys working with various age groups from the young athlete, to the weekend warrior, to the geriatric population.  Rhonda lives in a surrounding community with her husband and two sons.  Her family is of great importance to her.  They spend time together in the outdoors along with many days/weekends spent at sporting events including football and baseball.  Outside of time spent with her immediate family she enjoys time with her friends, being outdoors, and reading.







Kary Tracy, OT, earned a Bachelor’s of Science in Occupational Therapy in 2002. In addition to Brazosport Regional, Kary has worked for an early childhood intervention center, an Outpatient Rehab setting working with adults and the pediatric population, and a skilled nursing facility providing therapy services to geriatric patients. Kary has advanced training in upper extremity rehab, hand therapy, and custom splint fabrication. Her pediatric training includes Handwriting without Tears, Brain Gym, feeding therapy, fine motor skills, and sensory integration rehab. Kary lives in Lake Jackson with her husband and two kids who are involved with multiple sports and activities around the community.





Dr. Gail Yarborough, a Lake Jackson native, graduated from Texas Woman's University-Houston with her Doctorate in Physical Therapy in 2015. She joined the joined CHI St. Luke's Outpatient Rehab Clinic in 2016 (Brazosport Regional Health System at the time). She is certified Orthopedic Manual Therapy (COMT). She is currently pursuing  board certification in clinical orthopedics. She has taken several courses and pursuing Fellowship of the American Academy of Orthopedic Manual Therapy. She enjoys working with musculoskeletal orthopedic injuries, amputation and working towards lymphedema management. She believes in patient centric individualized treatment with focus on improving quality outcomes. She has retained membership in the American Physical Therapy Association since 2012.
