Diabetes Outpatient Training Site
CHI St. Luke's Health Brazosport is pleased to offer its Diabetes Outpatient Training Program, a program accredited by the American Association of Diabetes Educators
. The DOTS Program offers the latest information and instruction on diabetes care through our Diabetes Self-Management Course. The course is held in a series of four visits
, taught by a team including a Certified Diabetes Educator and a Registered Dietitian trained in diabetes care and management.
Session 1 – Individual Assessment
This is a one-to-one meeting with the diabetes educator. The educator will help you learn what is needed to take care of your diabetes. A major part of this session is learning to use your blood glucose meter. The educator will also perform a foot exam on you.
Session 2 – Disease Process and Nutrition 1
In this group class, you will learn what diabetes is and how to begin to manage it. The dietitian will teach you how to choose healthy foods, how to count carbohydrates and provide you with a personalized meal plan.
Session 3 – Exercise and Medication, Nutrition 2
Exercise programs and the different diabetes medications will be discussed in the third session. In addition, there will be important teaching on what to do when you are sick and when you are traveling. You will learn more about your meal plan in this group session.
Session 4 – Preventing Complications
During session four, you will receive information on how to keep complications from starting and/or getting worse.
In order to enroll in the program, you will need to have a doctor’s order. Have your physician/healthcare professional complete the Diabetes Services Order Form and fax it to the hospital fax number at the bottom of the form.
Most insurance companies recognize the importance of diabetes training. However, because insurance plans and coverage vary, we encourage you to find out if diabetes self-management training is covered by your policy and what your responsibility will be. If you do not have an insurance plan, other options may be available for you as well.
